Health First Chiropractic Clinic has a top priority of making sure that anyone can afford Chiropractic care. We accept all insurances but if you are one of the many Michiganders without health insurance benefits, or you have found that the insurance you have has limited coverage of Chiropractic care, please do not convince yourself that you cannot afford care. We offer plans that make an entire week of care less expensive that just one visit at other offices. We even offer the opportunity to come in for your First Visit for FREE, our web coupon offers you a consultation, examination,     2 plates of x-rays, first adjustment and report of findings, so all it costs for you to get informed about your condition is less than one hour of your time.

Chiropractic care is covered under many insurance plans. Most of our patients that have health or accident insurance will fall under one of the plans discussed in this policy. We have numerous ways to make your Chiropractic care affordable. We will work with you to find the right plan for you to receive the health care that you need.

 

Submitting Insurance Claims: For your convenience, we will submit a claim to your insurance company for services rendered whether we participate with your insurance company or not.

Group or Individual Insurance: We will call to verify benefits on your insurance. However, the benefits quoted to us by your insurance company are not a guarantee of payment. Payment will be due by you at the time of service for any non-covered services, co-pays or deductibles. Our fees are considered usual, customary and reasonable by most insurance companies and are therefore covered up to the maximum allowance determined by each carrier. This statement does not apply to insurance companies that reimburse based on an arbitrary schedule of fees bearing no relationship to the current standard of care in this area.

On the job” Injury (workers compensation): If you are injured on the job, your care should be paid for under your employer’s workers compensation insurance. You will need to inform your employer of the accident, have them sign the necessary authorization and obtain the name and address of the carrier of their insurance. If your employer does not provide us with this information, if a settlement has not been made within 3 months, or if you suspend or terminate care, any fees and services are due immediately.

Personal Injury or Automobile Accident: Please notify your auto insurance carrier of your visit to our office immediately. Notify our insurance department immediately if an attorney is representing you. Although you are ultimately responsible for your bill, we will wait for settlement of your claim for up to 6 months after you care is completed. Once the claim is settled or if you suspend or terminate care, any fees for services are due immediately.

Medicare: We do accept assignment from traditional Medicare. The check is usually sent directly to our office; in payment of the services that Medicare will cover, which for Chiropractors is only manual manipulation of the spine. Medicare pays for 80% of the allowable fee once the deductible has been satisfied. You are required to pay the deductible and the remaining 20%, as well as any non-covered services. You will be notified in advance before any non-covered procedures are performed. Patients with private Medicare plans may have different benefits than traditional coverage and will be reviewed with the patient as needed.

Secondary Insurance: Please inform us of any secondary insurance you may have. We will submit claims for you along with your primary insurance explanation of benefits.

Payment is expected at time of service: If your insurance company requires a co-payment or co-insurance (i.e. a $15 co-pay or 20% co-insurance), it is understood that you will make this payment at the time of service. Payments can be made on a weekly basis. Patients choosing to pay on a monthly basis must see Financial Adviser to set up the needed account information.

Statements: If there is a remaining balance on your account following notification to us by your insurance company, you will receive a statement in the mail. Patients choosing to not pay at the time of service will be charged the reasonable and customary charges for any services rendered, you will receive a statement in the mail. Please pay the balance within 15 days. Due to our automated billing system, sending your payment within 15 days avoids any additional collection activity. Statements not paid within 30 days, will begin to accrue interest at a rate of 0.5 per month. If you have questions regarding the determination of your health insurance coverage, please contact the office. A fee of $25 will be charged to your Health First Chiropractic account for checks returned to us for insufficient funds. When this occurs any future payments will need to be made by cash, money order or credit card.

Financial Assistance Policy: Patients who are uninsured or patients that are insured with high deductibles, co-pays, limits of coverage or whose insurance does not cover Chiropractic care at our office are personally responsible for payment. Payments may be made at the time of service for a discount. If for any reason you are not able to pay for services as they are rendered, we can assist you by setting up a payment plan. Please talk with our Financial Adviser. We want to help you continue to receive the health care that you need. Our clinic participates with CareNow; a payment plan that is not based on your credit score, all plans are interest free, and the payment amounts are based on what you can afford. We work hard to make sure that finances do not have to dictate your health.
